Our Support Coordination service helps you understand your plan, organise your supports and build the skills and confidence to have greater control over them.
The goal is not to take control of your plan. The goal is to help you take control.
How we can support you
Your Support Coordinator can assist you to:
Understand your NDIS plan
Understand your funded supports, how different budgets work and how your supports can be used towards your goals.
Connect with providers
Identify and connect with suitable NDIS providers, community organisations and mainstream services.
Set up your supports
Help organise services and work with providers so your supports operate effectively together.
Exercise choice and control
Provide information about available options so you can decide which providers and supports suit you.
Monitor your supports
Review whether your current supports are meeting your needs and identify issues before they become larger problems.
Build your independence
Develop your confidence and skills to understand, organise and direct your own supports where possible.
Respond when circumstances change
Assist when services break down, circumstances change or additional coordination is required.
Prepare for plan discussions
Help organise relevant information about your supports, goals, progress and ongoing needs.
Independent choice of providers
Camden Care may provide other NDIS services in addition to Support Coordination.
You are not required to use our organisation for any other support simply because we provide your Support Coordination.
Where we provide another service that may be relevant to you, we will disclose that interest.
You remain free to choose other providers.
Where appropriate, we can provide information about multiple suitable service options so you can decide what works best for you.
Who can use Support Coordination?
Support Coordination must be included in your NDIS plan for NDIS funding to be used for the service.
We can discuss your plan with you and determine whether the Support Coordination funding available matches the service you are seeking.
